Available Schedules:
📅 Saturday & Sunday:
- 7:00 AM - 3:30 PM
- 3:30 PM - 7:00 PM
Reliable transportation is required. Please indicate which schedule you are available for when applying.
Job Responsibilities:
- Personal Care: Assist with bathing, grooming, dressing, skincare, and mobility support
- Meal Preparation: Plan and prepare nutritious meals based on dietary needs
- Household Assistance: Maintain a clean, safe, and healthy environment for patients
- Documentation: Accurately record patient care activities using Celltrak
- Team Collaboration: Report changes in patient conditions to RN Case Managers
Requirements:
✔️ NJ Certified Home Health Aide (CHHA) in good standing
✔️ Ability to read, write, and communicate fluently in English
✔️ Physically able to lift, push, and pull up to 50 lbs
✔️ Willing to travel within Gloucester County
